Admissions Policy

Service-users may be admitted on either a long (without limit of time) or short term basis, or for
a period of assessment resulting in a written report and recommendations from Cascade Care about future care options. Service-users in this category may include persons in need of assessment prior to being accorded budget-holder status under the Personalisation Agenda.

Making a Referral
Referrals must be made in writing by a Social Worker or representative of the prospective service-user’s funding agency. In furtherance of our duty of care both towards both the person being referred and their funding agency, and in compliance with CQC requirements, we require full disclosure about each prospective service-user at the time of referral.

Receipt of required documentation also enables Cascade Care to carry out a detailed assessment of the service user at their current place of abode to identify appropriate risk management strategies and determine key areas of clinical interventions and care provision in the shortest possible time and so minimise the waiting period between referral and funding panel confirmation. Reports of the referral’s social and psychiatric history must accompany the Referral application Form, together with details of any offences.

Post assessment and pre funding panel, we communicate regularly with both the Referrer and the person being referred. All service users will by now have received their ‘Pre Admission Guide’ to living at Cascade that informs them about the service and introduces them to their prospective Home Manager.

Prior to admission, all service users are invited to make informal day visits and then stay overnight to meet staff and fellow residents.

Visits by Social Workers prior to making a Referral
Cascade welcomes visits to our Homes by Social Workers who are considering making a referral. We will arrange local transport for you and take you to the Home or Homes that are most appropriate to your service user’s needs.
