A Continuing Pathway of Care

Cascade Care service users begin their journey towards recovery on the day they join us.

To continue this journey, we recognise work with multi-disciplinary teams to plan a comprehensive pathway of care for service users when step down is agreed.

We also work with experienced providers of appropriate accommodation and support to help ensure that service users leaving Cascade Care continue their recovery journey.

The pathway to care is reinforced by our ongoing contact with service users during the three months after their move to supported living or independence. We stay in daily contact with each service user during the first week after their move and then on a weekly basis during this crucial settling in period.

The effectiveness of our holistic approach is demonstrated by our proven track record of service users successfully stepping down from our homes to independent, supported or semi-supported living.
